Distinct paths to alcohol problems: Impacts of childhood maltreatment, attachment insecurity, and interpersonal problems

Childhood maltreatment is a strong risk factor for increased alcohol use and alcohol-related problems. One possible explanation for these associations is the influence of dysfunctional attachment styles and interpersonal problems in adulthood. Individuals who have experienced childhood maltreatment are likely to develop maladaptive interpersonal styles due to insecure attachment. Maladaptive interpersonal styles may increase emotional distress, which in turn, can make these individuals vulnerable to alcohol use and alcohol-related problems. Using data from 508 undergraduate students, this study examined the associations between three types of childhood maltreatment (i.e., sexual abuse, punishment that included physical abuse, and a negative home atmosphere that included neglect), alcohol use, and alcohol-related problems via insecure attachment and interpersonal problems. The results showed that punishment predicted alcohol problems via avoidant attachment and selfish interpersonal style. Sexual abuse and negative home atmosphere predicted alcohol consumption through anxious attachment and selfless interpersonal style. In addition, sexual abuse directly predicted alcohol problems. These findings provide new insights for the field of childhood maltreatment and addiction literature.

Conjoint adolescent developmental trajectories of alcohol and marijuana use and early adult outcomes among North American Indigenous people

Objective: The current study examined the developmental interrelationships between alcohol and marijuana use trajectories from ages 10 to 18 years in a sample of North American Indigenous adolescents. Distinct co-use groups were formed to create profiles of young adult outcomes. Method: Dual group-based trajectory models of marijuana and alcohol frequency were estimated using data from a longitudinal community-based participatory study of Indigenous adolescents from the upper Midwest and Canada. Joint probabilities were used to create co-use groups, and profiles were created using early adult (Mean Age – 26.28 years) outcomes. Results: Four joint trajectory groups were identified: 1) no marijuana and no/low alcohol use (34.4%), 2) mid-onset alcohol only (14%), 3) mid-onset co-use starting at age 13 (24%), and 4) early-onset co-use starting at age 11 (22%). High probabilities existed that adolescents would use marijuana early if they began drinking alcohol at the youngest ages, and that adolescents would not use marijuana if they drank infrequently or delayed drinking until mid-adolescence. Adult outcomes were poorer for the early- and mid-onset co-use groups, but there were few differences between the no/low use and alcohol-only groups. Conclusion: Co-use of marijuana and alcohol was associated with poorer outcomes in early adulthood, particularly for the group with an earlier age of onset. Abstaining from either substance in adolescence was associated with better outcomes.

Fast-Track Liver Transplantation: Six-year Prospective Cohort Study with an Enhanced Recovery After Surgery (ERAS) Protocol

Introduction Enhanced recovery after surgery (ERAS) has been shown to facilitate discharge, decrease length of stay, improve outcomes and reduce costs. We used this concept to design a comprehensive fast-track pathway (OR-to-discharge) before starting our liver transplant activity and then applied this protocol prospectively to every patient undergoing liver transplantation at our institution, monitoring the results periodically. We now report our first six years results. Patients and methods Prospective cohort study of all the liver transplants performed at our institution for the first six years. Balanced general anesthesia, fluid restriction, thromboelastometry, inferior vena cava preservation and temporary portocaval shunt were strategies common to all cases. Standard immunosuppression administered included steroids, tacrolimus (delayed in the setting of renal impairment, with basiliximab induction added) and mycophenolate mofetil. Tacrolimus dosing was adjusted using a Bayesian estimation methodology. Oral intake and ambulation were started early. Results A total of 240 transplants were performed in 236 patients (191 male/45 female) over 74 months, mean age 56.3 +/- 9.6 years, raw MELD score 15.5 +/- 7.7. Predominant etiologies were alcohol (n = 136) and HCV (n = 82), with hepatocellular carcinoma present in 129 (54.7%). Nine patients received combined liver and kidney transplants. The mean operating time was 315 +/- 64 min with cold ischemia times of 279 +/- 88 min. Thirty-one patients (13.1%) were transfused in the OR (2.4 +/- 1.2 units of PRBC). Extubation was immediate (< 30 min) in all but four patients. Median ICU length of stay was 12.7 hours, and median post-transplant hospital stay was 4 days (2-76) with 30 patients (13.8%) going home by day 2, 87 (39.9%) by day 3, and 133 (61%) by day 4, defining our fast-track group. Thirty-day-readmission rate (34.9%) was significantly lower (28.6% vs. 44.7% p=0.015) in the fast-track group. Patient survival was 86.8% at 1 year and 78.6% at five years. Template-free method for the synthesis of high-pore-volume gamma-Al2O3 nanofibers in a membrane dispersion microreactor

Mesoporous gamma-Al2O3 nanofibers with high pore volume and uniform pore size distributions were successfully synthesized via a template-free method in a membrane dispersion microreactor followed by calcination. The effects of crystal temperature, pH values, continuous phase concentration and washing solvent on the gamma-Al2O3 nanofibers were carefully studied. The results showed that the as-obtained gamma-Al2O3 nanofibers showed a length of 40-60 nm and a width of 3.2-3.4 nm, which were attributed to the high microscopic mixing rate in the membrane dispersion microreactor. Moreover, the precursors of gamma-Al2O3 nanofibers treated with deionized water and mixed deionized water/alcohol solution had high pore volumes, reaching to 1.60 ml g(-1) and 2.00 ml g(-1), respectively. Furthermore, the adsorption performance of gamma-Al2O3 nanofibers with high pore volumes was also investigated. These fibers showed an excellent adsorption capacity of 1323.68 mg g(-1) for the removal of Congo red from aqueous solution, thereby indicating their potential for applications in adsorption and other related areas.

Red yeast rice prevents chronic alcohol-induced liver disease by attenuating oxidative stress and inflammatory response in mice

Alcoholic liver disease (ALD) is characterized by dyslipidemia, hepatic steatosis, steatohepatitis, edema, necrosis, etc. Studies have reported that some dietary nutrition factors have beneficial effects in improving ALD. Red yeast rice (RYR), a traditional herbal supplement, has been confirmed to lower cholesterol mainly due to its component monacolin K. However, the effect of RYR on ALD has not been investigated. In this study, mice were supplemented with a daily oral gavage of 4 g/kg 50% ethanol for 8 weeks to induce a chronic ALD. RYR (150 mg kg(-1) day(-1)) was supplied to ALD mice in treatment group. The results showed that RYR supplementation significantly attenuated hyperlipidemia, elevated circulating inflammatory cytokines, hepatic structural damage, and oxidative stress in mice supplemented with alcohol with no effects on body weight. Moreover, RYR significantly suppressed alcohol-induced hepatic NF-kappa B activation and apoptosis. Our results suggest that RYR is capable of preventing ALD mainly by attenuating hepatic oxidative stress and inflammatory response. Practical applications RYR was known for cholesterol-lowering effect through its main component monacolin K. The current study revealed that RYR was capable of ameliorating ALD, which is characterized by profound dyslipidemia, hepatic steatosis, steatohepatitis, edema, etc. Our results indicated that the protective effect of RYR on ALD is largely achieved by regulating lipid metabolism, and closely related to the anti-inflammatory and antioxidant effects of RYR. This study provides research foundation for the development of RYR-related food or pharmaceutical products, especially targeting for ALD.

Aerobic biological degradation of organic matter and fracturing fluid additives in high salinity hydraulic fracturing wastewaters

Reuse of hydraulic fracturing wastewaters depends on effective tailored treatment to prepare the water for the intended end use. Aerobic biological treatment of hydraulic fracturing produced waterwas examined to degrade dissolved organic carbon (DOC) and polyethylene glycols (PEGs). Biological treatment experiments of three produced water samples with DOC concentrations ranging from 22 to 420 mg/L and total dissolved solids (TDS) levels ranging from 26 to 157 g/L were conducted in 48-240 h batches. Samples were not pretreated to remove suspended solids and were inoculated with activated sludge and acclimated over several weeks. Results show that between 50% and 80% of DOC was removed in 12-24 h but a sizeable portion, on a mass basis, remained in the samples with higher DOC concentrations. PEGs were also shown to readily biodegrade into singly- and doubly-carboxylated metabolites, but were not shown to degrade past that point, leading to accumulation of PEG-dicarboxylates (PEG-diCs) in the batch reactors. Possible explanations include residence times that were too long, resulting in starved microbial populations (and thus, a stopping of PEG degradation) or the presence of other ethoxylated additives that degraded into PEGs and PEG-diCs and fed this accumulation. Thiswork demonstrates that a well-acclimated microbial culture is capable of degrading a large portion of DOC in hydraulic fracturingwastewaters across awide spectrum of TDS concentrations, indicating that biological treatment is a viable option for enabling reuse of produced water. Scalable De Novo Synthesis of Aldgarose and Total Synthesis of Aldgamycin N

Preparation and Characterization of Polyvinyl Alcohol-Chitosan/Cerium Hydrogel with Significant Antibacterial Activity

Traditional wound dressings have some major deficiencies, including poor antibacterial ability, low exudate absorption rate and dry environment. These defects can be addressed by the prepared hydrogels in this study, which can provide a moist and cool environment for wound healing. Polyvinyl alcohol-chitosan/cerium (PVA-Cs/Ce) composite hydrogels are prepared by a freeze-thaw method and characterized by x-ray diffraction (XRD) and scanning electron microscopy (SEM). In addition, the effects of chitosan/cerium (Cs/Ce) complexes with different contents on properties, including mechanical properties, water vapor permeability, swelling ratio, and thermal stability are investigated. SEM images indicate that the composite hydrogels have a good three-dimensional pore structure. Compared with the pure PVA hydrogel, the composite hydrogels show better swelling properties and more suitable water vapor transmission rate (2504 g m(-2).day) by adding Cs/Ce complexes. And the elongation at break of the hydrogel with a ratio of 5:3 (PVA: Cs/Ce) is the highest value of 567%. In the bacteriostatic circle experiment, PVA-Cs/Ce composite hydrogels exhibit a pronounced inhibitory effect against two bacteria (Staphylococcus aureus and Escherichia coli). These results suggest that these composite hydrogels have great potential as wound dressings.

Caffeine increases alcohol self-administration, an effect that is independent of dopamine D-2 receptor function

The rising popularity of alcohol mixed with energy drinks (AmEDs) has become a significant public health concern, with AmED users reporting higher levels of alcohol intake than non-AmED users. One mechanism proposed to explain this heightened level of alcohol intake in AmED users is that the high levels of caffeine found in energy drinks may increase the positive reinforcing properties of alcohol, an effect that may be dependent on interactions between adenosine receptor signaling pathways and the dopamine D-2 receptor. Therefore, the purpose of the current study was to confirm whether caffeine does increase the positive reinforcing effects of alcohol using both fixed ratio (FR) and progressive ratio (PR) designs, and to investigate a potential role of the dopamine D-2 receptor to caffeine-induced increases in alcohol self-administration. Male Long-Evans rats were trained to self-administer a sweetened alcohol solution (10% v/v alcohol + 2% w/v sucrose) on an FR2 schedule of reinforcement, and the effects of caffeine (0, 5, 10, and 20 mg/kg, i. p. [intraperitoneally1) on the maintenance of alcohol self-administration and alcohol break point were examined. Parallel experiments in rats trained to self-administer sucrose (0.8% w/v) were conducted to determine whether caffeine’s reinforcement-enhancing effects extended to a non-drug reinforcer. Caffeine pretreatment (5-10 mg/kg) significantly increased sweetened alcohol self-administration and motivation for a sweetened alcohol reinforcer. However, similar increases in self-administration of a non-drug reinforcer were not observed. Contrary to our hypothesis, the D-2 receptor antagonist eticlopride did not block a caffeine-induced increase in sweetened alcohol self-administration, nor did it alter caffeine-induced increases in motivation for a sweetened alcohol reinforcer. Taken together, these results support the hypothesis that caffeine increases the positive reinforcing effects of alcohol, which may explain caffeine-induced increases in alcohol intake. However, the reinforcement-enhancing effects of caffeine appear to be independent of D-2 receptor function. (C) 2020 Elsevier Inc. All rights reserved.

Patient Factors Associated With Opioid Consumption in the Month Following Major Surgery

Objective: The aim of this study was to determine preoperative patient characteristics associated with postoperative outpatient opioid use and assess the frequency of postoperative opioid overprescribing. Summary Background Data: Although characteristics associated with inpatient opioid use have been described, data regarding patient factors associated with opioid use after discharge are lacking. This hampers the development of individualized approaches to postoperative prescribing. Methods: We included opioid-naive patients undergoing hysterectomy, thoracic surgery, and total knee and hip arthroplasty in a single-center prospective observational cohort study. Preoperative phenotyping included self-report measures to assess pain severity, fibromyalgia survey criteria score, pain catastrophizing, depression, anxiety, functional status, fatigue, and sleep disturbance. Our primary outcome measure was self-reported total opioid use in oral morphine equivalents. We constructed multivariable linear-regression models predicting opioids consumed in the first month following surgery. Results: We enrolled 1181 patients; 1001 had complete primary outcome data and 913 had complete phenotype data. Younger age, non-white race, lack of a college degree, higher anxiety, greater sleep disturbance, heavy alcohol use, current tobacco use, and larger initial opioid prescription size were significantly associated with increased opioid consumption. Median total oral morphine equivalents prescribed was 600 mg (equivalent to one hundred twenty 5-mg hydrocodone pills), whereas median opioid consumption was 188 mg (38 pills). Conclusions: In this prospective cohort of opioid-naive patients undergoing major surgery, we found a number of characteristics associated with greater opioid use in the first month after surgery. Future studies should address the use of non-opioid medications and behavioral therapies in the perioperative period for these higher risk patients.
