A series of chiral α-alkylated ketones and macrolides were obtained with good yields and excellent enantioselectivities upon a sequence of decarboxylative benzylation, retro-Claisen cleavage, and enamine protonation. This strategy features broad substrate scope, mild conditions, as well as high atom economy with salicylic carbonates as the o-quinone methide precursors.

Chiral magnesium catalyzed intramol. vinylogous Michael reaction of novel cyclohexadienones via a desymmetrization process is reported. (R)-BINOL derived ligand and an achiral amide were employed in the current in situ generated magnesium catalyst, giving the corresponding hydrogenated benzofuranone skeletons in good to excellent enantioselectivities with high yields. This simple and efficient strategy could be utilized for the synthesis of aromatized α,β-unsaturated ester and Br-substituted hydrogenated benzofuranone in good yields under mild conditions.

Development of novel concepts to construct drug vehicles for efficient encapsulation of hydrophobic drugs and targeted therapy is highly desirable to biomedical science. It remains a challenge to create effective noncovalent linkage between the components within the drug vectors. In this work, we report a supramol. approach to construct multifunctional nanoparticles, which combined tumor-targeting capability via saccharide, cell penetration via pos.-charged surface, and chemotherapeutic function via drug delivery for cancer therapy. A endogenous self-associating nucleotide (guanosine monophosphate) noncovalently bonded with a hydrophobic nucleoside analog drug (clofarabine) to form nanofibrils, which were transformed to spherical nanoparticles by assembling with a fructose/ethanolamine-functionalized starlike poly(glycidyl methacrylate)-based cationic polymer. Noncovalent cavities within the nanoparticles were created, through the nucleotide self-association, and acted as linkage between the hydrophilic cationic polymer and the drug. The saccharide-tethering polycation and nucleotide components were labeled with fluorescent dyes, resp., to image the endocytosis and biodistribution. In vitro and in vivo results show that the functional nanoparticles exhibited good capabilities of the targeted accumulation of the nanoparticles and delivery of drugs in cancer cells and tumor sites, leading to a robust anticancer effect.

Objective: Triflusal presents comparable antiplatelet activity to aspirin while presenting a more favorable safety profile, and is used in the treatment of thrombosis. The study aimed to evaluate the pharmacokinetics and safety of triflusal and its major metabolite 2-(hydroxyl)-4-(trifluoromethyl)- benzoic acid (HTB) in healthy Chinese subjects. Methods: 30 healthy subjects were recruited in this randomized, single-center, and open-label, parallel, single ascending doses (300, 600, 900 mg) and multiple doses (600 mg, once daily for 7 days) study. Plasma samples were analyzed with a validated liquid chromatog. tandem mass spectrometry (LC/MS/MS) method. Safety was assessed by adverse events, ECG, laboratory testing, and vital signs. Results: Triflusal was safe and well tolerated. After single-dose administration, triflusal was rapidly absorbed with a mean Tmax of 0.55-0.92 h and a mean t1/2 kel of 0.35-0.65 h, HTB was absorbed with a mean Tmax of 2.35-3.03 h and a mean t1/2 kel of 52.5-65.57 h. Cmax and AUC for triflusal and HTB were approx. dose proportional over the 300-900 mg dose range. In the steady state, the accumulation index (R) indicated that the exposure of triflusal increased slightly with repeated dosing, and the exposure of HTB increased obviously. 3 Adverse events certainly related to the investigational drugs occurred in the multiple-dose phase. Conclusion: Following oral dosing under fasting condition, triflusal is promptly absorbed and rapidly depleted from the systemic circulation. HTB is quickly generated from triflusal and slowly eliminated. Triflusal accumulates slightly in the body. HTB plasma concentration builds up progressively toward steady-state.

Folate derivatives are crucial growth factors for Ketogulonigenium vulgare which is used in mixed culture with Bacillus megaterium for the industrial production of 2-keto-L-gulonic acid (2-KGA), the precursor of L-ascorbic acid (L-AA) or vitamin C (Vc). To improve the growth and 2-KGA production, five genes involved in folate biosynthesis identified in a folate gene cluster from Lactococcus lactis MG1363, including folB, folKE, folP, folQ and folC, were over-expressed in K. vulgare. Intracellular folate concentration in the recombinant strain harboring folate biosynthesis genes cluster under the control of Psdh (sorbose dehydrogenase gene sdh promoter from K. vulgare) was 8 times higher than that of the wildtype K. vulgare DSM 4025 (P < 0.001). In shake flask studies, the cell d. and 2-KGA production of the recombinant K. vulgare Rif (pMCS2PsdhfolBC) were increased by 18% (P < 0.001) and 14% (P < 0.001), resp., under a relatively stable pH 7 condition. In fermentor studies, enhancements around 25% cell d. (P < 0.001) and approx. 35% 2-KGA productivity (P < 0.001) were observed in comparison with the controls without over-expressing the folate biosynthesis genes. This was the first successful study of metabolic engineering on K. vulgare for enhanced 2-KGA production

The aim of this paper is to investigate the clin. efficacy of oxytocin combined with prostaglandins in the prevention of postpartum hemorrhage of high-risk pregnancy puerpera with caesarean section. High-risk pregnancy puerpera(330 cases) with caesarean section in Affiliated Hospital of Yan’an University from Jan. 2012 to Dec. 2013 were randomly divided into 3 groups(110 cases in each group): carboprost tromethamine+oxytocin(CTO) group, carboprost methylate+oxytocin(CMO) group, and oxytocin group. And patients in the CTO group were i.m. given carboprost tromethamine 250 μg in the body of uterus following fetus expulsion, and then were iv oxytocin 10 U. The patients in the CMO group were sublingual administered with carboprost methylate 1 mg following fetus expulsion, and were iv oxytocin 10 U. The patients in the oxytocin group were iv oxytocin 10 U following fetus expulsion, and were i.m. given oxytocin 10 U in body of uterus. The quantity of bleeding was compared among the three groups during the operation, 2, and 2-4 h after operation. The postpartum hemorrhage, blood transfusion, and addnl. hemostatic measures after delivery were observed among the three groups. The side reaction was also observed among the three groups. During operation, 2, and 2-4 h after operation, the quantity of bleeding of maternal in the CTO group was less than that in the CMO group and oxytocin group, with the significant difference(P<0.05). During the operation, 2 h after operation, the quantity of bleeding of maternal in the CMO group was significantly less than that in the oxytocin group(P<0.05). While 2-4 h after the operation, there was no significant difference. The postpartum hemorrhage, blood transfusion rate, and addnl. intervention measures to stop bleeding in the CTO and CMO groups were significantly lower than those in the oxytocin group(P<0.05). The side effects of the three groups were 9.09%, 8.18%, and 6.36%, resp., and there was no significant difference. CTO is effective in preventing postpartum hemorrhage of high-risk pregnancy. CMO could reach a certain preventive effect, but the application of oxytocin simply showed poor efficacy.

Corn steep liquor (CSL) is one of the main raw materials in 2-keto-L-gulonic acid (2-KLG) fermentation by Ketogulonicigenium vulgare and Bacillus megaterium. The role of ambroxol hydrochloride combined with fiberoptic bronchoscopy in elderly patients with severe pneumonia remains unclear, we aimed to analyze this issue to provide evidences into the management of clin. pneumonia. We searched PubMed et al databases up to Oct. 20, 2021 for the randomized controlled trials on the application of ambroxol hydrochloride combined with fiberoptic bronchoscopy in elderly patients with severe pneumonia. Related outcomes were extracted and analyzed. Review Manager 5.3 software was used for data anal. A total of 13 randomized controlled trials involving 1317 elderly patients (559 cases in the ambroxol hydrochloride+fiberoptic bronchoscopy group and 658 cases in the fiberoptic bronchoscopy group) with pneumonia were included. Meta-analyses indicated that the blood oxygen partial pressure [mean difference (MD)=5.75, 95% confidence interval (CI) (3.80, 7.70)], blood oxygen saturation [MD=6.43, 95% CI (4.39, 8.48)], oxygenation index [MD=26.75, 95% CI (14.61, 38.89)] of exptl. group was significantly higher than that of control group (all P001), the incidence of multiple organ failure [odds ratio=0.42, 95% CI (0.31, 0.56), P001], mortality on day 28 [odds ratio=0.44, 95% CI (0.33, 0.59)] of exptl. group was significantly less than that of control group (all P001). The high-dose ambroxol hydrochloride combined with fiberoptic bronchoscopy is beneficial to improve the patients blood gas indicators, and reduce mortality in elderly patients with severe pneumonia.

In this article, we report the synthesis of poly(benzyl methacrylate) (PBzMA) latexes via reversible addition-fragmentation chain-transfer (RAFT) mediated miniemulsion polymerization using non-ionic Lutensol TO 20 surfactant. The influence of hydrophobe, initiator, and RAFT agent on particle diameter, particle number, rate of polymerization, and molar mass were investigated systematically. NAMPT inhibitors may show potential as therapeutics for oncol. Throughout our NAMPT inhibitor program, we found that exposed pyridines or related heterocyclic systems in the left-hand portion of the inhibitors are necessary pharmacophores for potent cellular NAMPT inhibition. However, when combined with a benzyl group in the center of the inhibitors, such pyridine-like moieties also led to consistent and potent inhibition of CYP2C9. In an attempt to reduce CYP2C9 inhibition, a parallel synthesis approach was used to identify central benzyl group replacements with increased Fsp3. A spirocyclic central motif was thus discovered that was combined with left-hand pyridines (or pyridine-like systems) to provide cellularly potent NAMPT inhibitors with minimal CYP2C9 inhibition. Further optimization of potency and ADME properties led to the discovery of compound 68, a highly potent NAMPT inhibitor with outstanding efficacy in a mouse tumor xenograft model and lacking measurable CYP2C9 inhibition at the concentrations tested.
