Building evidence suggests that dihydromyricetin (DHM), a bioactive flavonoid isolated from Hovenia dulcis, provides hepatoprotection by enhancing ethanol metabolism in the liver by maintaining hepatocellular bioenergetics, reductions of oxidative stress, and activating lipid oxidation pathways. The present study investigates the utility of DHM on hepatic mitochondrial biogenesis via activation of the AMP-activated protein kinase (AMPK)/Sirtuin (Sirt)-1/PPARG coactivator 1 (PGC)-1 alpha signaling pathway. We utilized a forced drinking ad libitum study that chronically fed 30% ethanol to male C57BL/61 mice over 8 weeks and induced ALD pathology. We found that chronic ethanol feeding resulted in the suppression of AMPK activation and cytoplasmic Sirt-1 and mitochondrial Sirt-3 expression, effects that were reversed with daily DHM administration (5 mg/kg; intraperitoneally [i.p.]). Chronic ethanol feeding also resulted in hepatic hyperacetylation of PGC-1 alpha, which was improved with DHM administration and its mediated increase of Sirt-1 activity. Furthermore, ethanol-fed mice were found to have increased expression of mitochondrial transcription factor A (TFAM), reduced mitochondrial content as assessed by mitochondrial DNA to nuclear DNA ratios, and significantly lower levels of hepatic ATP. In contrast, DHM administration significantly increased TFAM expression, hepatic ATP concentrations, and induced mitochondrial expression of respiratory complex III and V. In total, this work demonstrates a novel mechanism of DHM that improves hepatic bioenergetics, metabolic signaling, and mitochondrial viability, thus adding to the evidence supporting the use of DHM for treatment of ALD and other metabolic disorders. Patient Factors Associated With Opioid Consumption in the Month Following Major Surgery

Objective: The aim of this study was to determine preoperative patient characteristics associated with postoperative outpatient opioid use and assess the frequency of postoperative opioid overprescribing. Summary Background Data: Although characteristics associated with inpatient opioid use have been described, data regarding patient factors associated with opioid use after discharge are lacking. This hampers the development of individualized approaches to postoperative prescribing. Methods: We included opioid-naive patients undergoing hysterectomy, thoracic surgery, and total knee and hip arthroplasty in a single-center prospective observational cohort study. Preoperative phenotyping included self-report measures to assess pain severity, fibromyalgia survey criteria score, pain catastrophizing, depression, anxiety, functional status, fatigue, and sleep disturbance. Our primary outcome measure was self-reported total opioid use in oral morphine equivalents. We constructed multivariable linear-regression models predicting opioids consumed in the first month following surgery. Results: We enrolled 1181 patients; 1001 had complete primary outcome data and 913 had complete phenotype data. Younger age, non-white race, lack of a college degree, higher anxiety, greater sleep disturbance, heavy alcohol use, current tobacco use, and larger initial opioid prescription size were significantly associated with increased opioid consumption. Median total oral morphine equivalents prescribed was 600 mg (equivalent to one hundred twenty 5-mg hydrocodone pills), whereas median opioid consumption was 188 mg (38 pills). Conclusions: In this prospective cohort of opioid-naive patients undergoing major surgery, we found a number of characteristics associated with greater opioid use in the first month after surgery. Future studies should address the use of non-opioid medications and behavioral therapies in the perioperative period for these higher risk patients.

Background: The growing environmental concern of plastic packaging disposal has led to the innovation of biodegradable biopolymers. Whereas, the use of chemical dyes as an indicator in smart packaging is not suitable for food packaging because of their high toxicity and harmful effects on human health and the environment. Hence, the researchers are focused on natural pigments derived from plants and food waste as indicating substance in biodegradable packaging and also for the valorization of food waste. Scope and approach: This paper summarizes the research on the utilization of naturally derived food- and food waste-based pigments (anthocyanins, curcumin, betalains, carotenoids, chlorophyll, brazilin, quercetin, etc.) with biopolymeric matrices (starch, cellulose, chitin, gums, agar, etc.) to fabricate smart biodegradable films, for effective monitoring of spoilage and quality of meat products, seafood, milk, and others. Key Findings and Conclusions: The results show that the smart packaging material developed by the biopolymers with plant-based pigment has the potential to replace the traditional plastic packaging materials. The extracted from food and food waste act as an indicator in smart packaging and promotes the valorization of food waste. The biodegradable packaging is economical, safe, non-toxic, sensitive, and natural pigments act as a quality indicator in packaging systems. PEO/PVA/LiOH Solid Polymer Electrolyte Prepared via Ultrasound-assisted Solution Cast Method

Most solid polymer electrolytes (SPEs) still suffer from low ionic conductivity at room temperature, which may be caused by the inhomogeneity of polymer blend and Li-salt. Here, polyethylene oxide (PEO), polyvinyl alcohol (PVA), and lithium hydroxide (LiOH) SPEs were prepared by an ultrasonic method to improve the ionic conductivity and shorten the synthesis time. This procedure has successfully yielded a flexible, transparent, and homogeneous SPEs, which were thermally stable at fairly high temperature as revealed from thermogravimetric analysis (TGA). These samples also showed relatively high ionic conductivity at room temperature with the highest conductivity of 1.25 x 10(-4) S/cm and low activation energy of 0.253 eV demonstrated by sample with 2 wt% LiOH loading. Moreover, this sample was also stable in the measured voltage range, i.e., -5 to 5 V. DFT calculation showed that strong intermolecular charge transfer was present in the interaction between Li+ ion with PVA and PEO chain.

Managing comorbid patients in a psychiatric facility is a delicate matter that requires a collaborative team. The aim of this systematic paper is to highlight the following: The possibility of treating alcohol use disorder (AUD) and alcohol withdrawal syndrome (AWS) overlapping alcohol liver disease (ALD) within a psychiatric institution, and the importance of a collaborative multidisciplinary team; correctly dosing psychoactive medication when metabolism is affected by ALD; deciding when is it necessary to seek a transfer to a general hospital. Prescribing medication in patients suffering from ALD is still a not a fully documented territory. Protein binding, metabolism, bioavailability, extraction ratios, excretion route, and half-life must be taken into consideration as well as frequently repeating liver panels. Studies suggest that short-acting benzodiazepines are preferred over their alternatives when treating AWS in ALD. All anticonvulsants can be used in patients with decompensated liver disease with caution, although newer generation antiepileptic agents should be first line. Propofol is favored to benzodiazepines or opioids in the case of decompensated cirrhosis. Patients with ALD are likely to be further compromised by the potential hepatocytotoxicity of some pharmacological agents. On that account, having an integrated perspective of the medical case while taking into consideration the underlying illness as well as possible drug interaction is crucial in treating AUD or AWS in a psychiatric institution.

Distinct paths to alcohol problems: Impacts of childhood maltreatment, attachment insecurity, and interpersonal problems

Childhood maltreatment is a strong risk factor for increased alcohol use and alcohol-related problems. One possible explanation for these associations is the influence of dysfunctional attachment styles and interpersonal problems in adulthood. Individuals who have experienced childhood maltreatment are likely to develop maladaptive interpersonal styles due to insecure attachment. Maladaptive interpersonal styles may increase emotional distress, which in turn, can make these individuals vulnerable to alcohol use and alcohol-related problems. Using data from 508 undergraduate students, this study examined the associations between three types of childhood maltreatment (i.e., sexual abuse, punishment that included physical abuse, and a negative home atmosphere that included neglect), alcohol use, and alcohol-related problems via insecure attachment and interpersonal problems. The results showed that punishment predicted alcohol problems via avoidant attachment and selfish interpersonal style. Sexual abuse and negative home atmosphere predicted alcohol consumption through anxious attachment and selfless interpersonal style. In addition, sexual abuse directly predicted alcohol problems. These findings provide new insights for the field of childhood maltreatment and addiction literature.

Reducing the destructive effect of ambient humidity variations on gas detection capability of a temperature modulated gas sensor by calcium chloride

Despite the successful detection performance of electronic nose in laboratories, they face challenges for use in the industry due to their accuracy reduction resulted from variable ambient properties especially relative humidity (RH) variation which is studied here. Responses of a single temperature modulated metal oxide gas sensor have been analyzed by SVM and k-NN methods to achieve a detector for low concentration levels of acetone, ethanol, 1-propanol and 1-butanol in air. The classifier models were designed and tested under different train-test conditions which showed that studied gases can be detected by the classifier if only they were measured in the same train and test environmental conditions and deviation of humidity level from train condition, reduces the detection accuracy to less than 60%. The accuracy increases by expanding the training dataset and training the system with responses carried out for gas with various RH contents. It was also shown that by using CaCl2 at the rout of gas flow, the destructive effect of RH variation is reduced and the detection accuracy increases to above 90%, while to achieve this accuracy, it is not necessary to train the system in all humidity conditions. By this method, the number of required test for system training reduces drastically. The method can be generalized to other electronic nose and gas detectors which suffer from humidity variations.
