Objectives: To compare the effectiveness of zero-calorie soft drink and plain water as a solvent for sodium phosphate in terms of good palatability and better patient tolerance. METHODS: The randomised controlled trial was conducted from May to December 2019 at the Dowites Operation Theatre Endoscopy Suite, Surgical Unit 3, Civil Hospital Karachi, and comprised patients aged >18 years of either gender undergoing colonoscopy for screening and non-emergency/non-urgent colorectal diseases. The patients were randomised into group A, which was assigned to take sodium phosphate in water, and group B, which was assigned to take sodium phosphate in zero-calorie soft drink. Bowel preparation was assessed by the consultant during endoscopy. Outcome variables, such as bowel cleanliness, palatability, tolerance of solution, adverse effects, and willingness to repeat the preparation, were evaluated in both groups. Data was analysed using SPSS 21. RESULTS: Of the 162 patients, there were 81(50%) in each of the two groups. There were 124(76.5%) males and the overall mean age was 43±8.66 years. The palatability score was significant (p=0.01) for group B compared to group A. Due to better palatability and tolerance, 64(79%) patients in group B took the preparation in <6 hours. CONCLUSIONS: Use of zero-calorie soft drink was found to be a better option for colonoscopic preparation compared to plain water. The N-linked glycosylation pattern is an important quality attribute of therapeutic glycoproteins. It has been reported by our group and by others that different carbon sources, such as glucose, mannose and galactose, can differently impact the glycosylation profile of glycoproteins in mammalian cell culture. Acting on the sugar feeding is thus an attractive strategy to tune the glycan pattern. However, in case of feeding of more than one carbon source simultaneously, the cells give priority to the one with the highest uptake rate, which limits the usage of this tuning, e.g. the cells favor consuming glucose in comparison to galactose. We present here a new feeding strategy (named ‘TAFE’ for targeted feeding) for perfusion culture to adjust the concentrations of fed sugars influencing the glycosylation. The strategy consists in setting the sugar feeding such that the cells are forced to consume these substrates at a target cell specific consumption rate decided by the operator and taking into account the cell specific perfusion rate (CSPR). This strategy is applied in perfusion cultures of Chinese hamster ovary (CHO) cells, illustrated by ten different regimes of sugar feeding, including glucose, galactose and mannose. Applying the TAFE strategy, different glycan profiles were obtained using the different feeding regimes. Furthermore, we successfully forced the cells to consume higher proportions of non-glucose sugars, which have lower transport rates than glucose in presence of this latter, in a controlled way. In previous work, a math. model named Glycan Residues Balance Anal. (GReBA) was developed to model the glycosylation profile based on the fed carbon sources. The present data were applied to the GReBA to design a feeding regime targeting a given glycosylation profile. The ability of the model to achieve this objective was confirmed by a multi-round of leave-one-out cross-validation (LOOCV), leading to the conclusion that the GReBA model can be used to design the feeding regime of a perfusion cell culture to obtain a desired glycosylation profile.

Cf. Holtappel, C. A. 22, 1214. The original Dauphin method is as follows: weigh 2 g. oil in a 100-cc. flask, add 20 cc. of 5% alc. NH2OH.HCl previously neutralized to helianthin, immediately titrate the liberated HCl with 0.2 N KOH in presence of helianthin. Holtappel’s modifications consist essentially in cooling the solution with ice, use of excess alkali with back titration after 15 min., and use of bromothymol blue instead of helianthin. Investigation of the effects of temperature showed negligible variations (maximum 0.5%) from 0° to 35°. Investigation of the causes of the irregularities in results obtained by Holtappel brought out the following facts: (1) citronellal is cyclicized by strong acids into isopulegol and its derivatives, there being formed chiefly hydroxy-3-ethoxy-8-p-menthane in alc. solution, while in hydro-alc. solution there is formed a mixture of this compound with 3,8-p-menthane diol; (2) neither of these compounds combines with NH2OH.HCl, and their formation leads to low results; (3) in strongly alc. solution these reactions are of secondary importance, there being formed citronellal ethylacetal, which protects the aliphatic mol. against cyclization; (4) contrary to generally accepted data, this reaction is almost instantaneous, even at -15°; (5) slight acidity catalyzes oximation before cyclization can take place to any appreciable extent. It is concluded that: (1) helianthin should be used as indicator, as the turning point of bromothymol blue is at such a low pH value of the solution that appreciable cyclization can occur, while a light red color with helianthin indicates optimum conditions for oximation and absence of cyclization; (2) the alc. KOH should be added sufficiently quickly to avoid production for any appreciable time of the distinct red characteristic of relatively high acidity, and sufficiently slowly to prevent high alkalinity which might slow the reaction and even prevent it from going to completion through formation of the fairly stable acetal. The fact that Holtappel obtained lower results by oximation in alk. solution than by oximation at low temperature in acid solution is attributed to the fact that, because of the use of bromothymol blue instead of helianthin, the true effects of acidity could not be detected. The original Dauphin method is more simple and rapid than Holtappel’s modification and is as accurate as the latter. The method is being applied in the Dauphin Laboratories to the determination of ketones in all essential oils (the determination of citronellal being the most delicate); and where the speed of the reaction is relatively low, by plotting curves the presence of several ketones in the same oil can frequently be brought out very clearly. The method is particularly suitable for determination of menthone in oil of mint, instead of the usual method of reducing with Na, washing, acetylating and saponifying

Antifungal activity of thyme (Thymus vulgaris) essential oil (EO) in vapor-phase was tested against representative fungi in corn tortillas. The chem. composition of studied EO was analyzed by gas chromatog.-mass spectroscopy, and its major components were linalool, thymol, and p-cymene. The antifungal activity was evaluated by determining the growth of Aspergillus niger or Penicillium expansum after exposure to EO vapors. The in vitro min. inhibitory concentration (MIC) of the EO was determined by the inverted lid technique, while in situ MIC was determined on the corn tortillas inside an airtight container. The MICs obtained ranged from 160 to 200μL of thyme EO/Lof air for in vitro conditions and 550-850μL of the EO/Lof air in corn tortillas. The modified Gompertz model adequately described in vitro mold growth curves. Thyme EO was effective in preventing or significantly delaying growth of the contaminating molds on corn tortillas.

PURPOSE: The purpose of this study was to evaluate whether patients undergoing primary total hip arthroplasty (THA) at a higher elevation have higher rates of: 1) in-hospital length of stay (LOS); 2) surgical site infections (SSIs); 3) periprosthetic joint infections (PJIs); and 4) costs. MATERIALS AND METHODS: The Medicare claims database was used to identify patients who underwent primary THA at an altitude higher than 4,000 feet (ft). High-altitude patients were matched to patients who underwent primary THA at an altitude less than 100 ft, yielding 24,958 and 124,765 patients respectively. Outcomes that were assessed included in-hospital LOS, SSIs, PJIs, and costs of care. A P-value less than 0.001 was considered statistically significant. RESULTS: Patients in the study group had significantly longer in-hospital LOS (4 days vs 3 days, P<0.0001). In addition, patients in the study group had a significantly higher incidence and odds of developing SSIs (1.16% vs 0.86%; odds ratio [OR], 1.34; P<0.0001) and PJIs (0.91% vs 0.58%; OR, 1.56; P<0.0001) within 90-days following the index procedure, compared to matched controls. Higher altitude patients incurred higher day of surgery ($16,139.76 vs $15,279.42; P<0.0001) and 90-day costs ($18,647.51 vs $16,401.62; P<0.0001). CONCLUSION: This study demonstrated that primary THA procedures performed at an elevation higher than 4,000 ft are associated with longer in-hospital LOS, higher rates of SSIs, PJIs, and costs of care. Orthopedists and other healthcare professionals can use this information to provide adequate education for these patients regarding the potential complications that may occur following their procedure. The aim of the present research was to determine the chem. composition and the cyto- toxic effects of Tetraclinis articulata trunk bark essential oil (HEE) obtained by steam distillation and five fractions obtained by normal phase silica chromatog. separation Chem. anal. al- lowed the identification of 54 known compounds Relatively high amounts of oxygenated sesquiterpenes (44.4-70.2%) were detected, mainly consisting of caryophyllene oxide (13.1-26.6%), carotol (9.2-21.2%), 14-hydroxy-9-epi-(E)-caryophyllene (3.2-15.5%) and humulene epoxide II (2.6-7.2%). The cytotoxic activity against human mammary carcinoma cell lines (MDA-MB-231) and colorectal carcinoma cell lines (SW620) of the essential oil and its fractions were assessed. All the samples displayed moderate to weak activity compared to 5-fluorouracil. The colorectal carcinoma cell line was relatively more sensitive to the essential oil and its fractions compared to the breast cancer cell line, showing IC50 values from 25.7 to 96.5μg/mL. In addition, the essential oil and its fraction E.2 revealed a cytotoxic activity against colorectal carcinoma cell line, with IC50 values lower than 30μg/mL. This is the first report on the chem. composition and cytotoxic activity of the trunk bark essential oil of T. articulata.

The authentication and traceability of spices is a major concern for industrials and consumers. We focused on species from Zanthoxylum genera which are used for many different applications by local populations and also for trading as spices (dried pericarps or whole fruits). In this case, literature gives contradictory data about botanical names, and com. labeling is often confusing. We studied com. fruits pericarps extracts obtained by supercritical CO2 and analyzed them by GC/MS. The very complex volatile and semi volatile fractions composition of each extract is described. The barcoding method including mol. biol. and phylogenetic analyses was also developed in order to check the com. botanical identification of the raw material. This is a robust method to identify species in berries samples. We used one genetic marker to identify two Rutaceae clusters, including several species of Zanthoxylum genus. These results indicate that Fagara and Zanthoxylum groups could be considered as two different genera. Combination of chem. anal. and DNA anal. provides an original approach to increase chem. and botanical Zanthoxylum genus knowledge.

Phosphoglucomutase 1 deficiency is a congenital disorder of glycosylation (CDG) with multiorgan involvement affecting carbohydrate metabolism, N-glycosylation and energy production The metabolic management consists of dietary D-galactose supplementation that ameliorates hypoglycemia, hepatic dysfunction, endocrine anomalies and growth delay. Previous studies suggest that D-galactose administration in juvenile patients leads to more significant and long-lasting effects, stressing the urge of neonatal diagnosis (0-6 mo of age). Here, we detail the early clin. presentation of PGM1-CDG in eleven infantile patients, and applied the modified Beutler test for screening of PGM1-CDG in neonatal dried blood spots (DBSs). All eleven infants presented episodic hypoglycemia and elevated transaminases, along with cleft palate and growth delay (10/11), muscle involvement (8/11), neurol. involvement (5/11), cardiac defects (2/11). Standard dietary measures for suspected lactose intolerance in four patients prior to diagnosis led to worsening of hypoglycemia, hepatic failure and recurrent diarrhea, which resolved upon D-galactose supplementation. To investigate possible differences in early vs. late clin. presentation, we performed the first systematic literature review for PGM1-CDG, which highlighted respiratory and gastrointestinal symptoms as significantly more diagnosed in neonatal age. The modified Butler-test successfully identified PGM1-CDG in DBSs from seven patients, including for the first time Guthrie cards from newborn screening, confirming the possibility of future inclusion of PGM1-CDG in neonatal screening programs. In conclusion, severe infantile morbidity of PGM1-CDG due to delayed diagnosis could be prevented by raising awareness on its early presentation and by inclusion in newborn screening programs, enabling early treatments and galactose-based metabolic management.
